Pennsylvania Resource for Continuity of Care in Youth Serving Systems and Transitions

(PRCCYSST)- “PERSIST”

This is the 4th grant that Pennsylvania has received from the SAMHSA for youth suicide prevention in the Commonwealth. For this grant, we chose to base our acronym on the word persist, because although suicide prevention can be complex, we as a team, and those who care for youth at risk, continue to persist, because each life is worth it. Our approach to “Persist” involves two tiers. Tier 1 builds upon our prior and existing grant work focused on developing and sustaining suicide prevention efforts across the entire commonwealth, specifically in schools, colleges, and primary care settings.

Tier 2 engages target counties to improve continuity of care for youth at increased risk for suicide. Our continuity of care model will be systematically rolled out across 15 counties over the course of the project. The elements of the Zero Suicide framework will serve as an organizing structure for our targeted approach.  To learn more about the project and how we are utilizing Zero Suicide, please view the video below.
